How do I create a text box to add on the Consultation Forms?

Menu > Consultation Forms > here you can either Edit a form or Add a Form

Pick which page you'd like to add the box in e.g. 2 and scroll down to the bottom of the page > press Add Item:

Heading - Creates a Title for your Page

Field -  This where you add your question, a placeholder is where you tell a client what you want them to add here. e.g. "please list all medication you are currently taking"

Paragraph - Allows a client to free type information

Select - allow multiple choice 

Checkbox - adds a tick box option for clients

Separator - adds a line

Space - add a space between the question

Signature - will add a finger signature for clients to sign forms

Press Save
